Outpatient Physical Therapist

Outpatient Physical Therapist Job Description :

  • Develops a personalized care plan and implements treatment based on evaluation results and patient-specific functional goals.
  • Continuously monitors and assesses treatment outcomes, adjusting the plan as needed based on the patient's progress. Provides status updates to physicians and addresses factors impacting the patient's physical and psychosocial well-being.
  • Coordinates patient discharge, including creating home exercise programs, educating patients and families, arranging referrals for community services, and facilitating necessary equipment orders.
