About the Facility
Hopi Health Care Center, Polacca, Arizona
The Hopi Health Care Center, located in Polacca, Arizona, was built on the Hopi Reservation and serves approximately 7,000 people from both the Hopi and Navajo tribes. The Service Unit site was selected for its central accessibility, where most residents can reach the Center within 35-40 minutes. The Center is JCAHO-accredited, providing innovative health care services to the area's residents. The ambulatory care setting offers primary and preventative medical services, while the facility offers a 2-bed birthing unit, a 4-bed Medical / Surgical adult / pediatric unit supported by a staff of 12 physicians and 3 physician extenders. Services provided include general family medical care, surgical follow-up, pediatric and obstetric services. An eye clinic, staffed by 2 optometrists, provides refraction and general eye care. Seven dentists are stationed at the Center and at a satellite clinic providing dental services. Major trauma, acute surgical emergencies, and high-risk obstetric patients are transported to Tuba City Indian Medical Hospital, Chinle Hospital, Phoenix Indian Medical Center, or to several other contracted health facilities.

Registered Nurse - RESPONSIBILITIES (listed are included but not limited to)
Receive, initiate, and carry out medical orders placed by the provider : administer medications, oxygen, and a variety of treatments.
Manage a wide variety of therapeutic devices such as drainage, feeding, and suction tubes, also respiratory and electrocardiograph machines, and other medical equipment and supplies.
Measure and assess vital signs, height, weight, and neurological status, effects of medication, visual acuity, and hearing.
Observe and document patient care provided, changes in the patient's conditions, observations, and other designated unit activities in the accepted format and form.
Collaborate and coordinate patient care with other health care providers and hospital staff.
Working knowledge of adult and pediatric immunization schedules and administration practices
Perform quality control for lab checks, capillary blood glucose monitoring checks, etc.
Collect or assist in the collection of specimens, i.e., blood, urine, stool, etc.
Recognize life-threatening emergencies, initiate and maintain resuscitation measures and other emergency procedures as appropriate and in collaboration with the medical provider, recognize adverse reactions to medications, treatments, and procedures, and initiate interventions in collaboration with the medical provider.
Answer and screen telephone calls.
Registered Nurse - REQUIREMENTS
Education : Must have completed an accredited nursing program at the associate or bachelor's level.
Licensure : Must possess a current, valid, unrestricted nursing license in a state, the District of Columbia, the Commonwealth of Puerto Rico, or a Territory of the United States.
Certification : Valid American Heart Association certifications in Basic Life Support (BLS), Advanced Cardiovascular Life Support (ACLS), and Pediatric Advanced Life Support (PALS).
Experience : A minimum of two (2) years of nursing experience, additional urgent care experience is a plus.
