Job Summary : Licensed Vocational Nurse (LVN)
American Hospice and Home Health Services is seeking a dedicated Part-Time or Per Diem On-Call LVN to serve patients in hospice care. This position is focused on improving the quality of life for patients and supporting their families during difficult times.

Essential Job Functions / Responsibilities :
As a Hospice and Home Health LVN, you will :
Provide nursing care and supportive services following the physician / provider treatment plan.Conduct thorough assessments, gather pertinent data, and develop care plans.Monitor patient progress, document observations, and implement care plans effectively.Collaborate with team members to deliver optimal care and services.Complete all documentation in compliance with policies and procedures.Communicate effectively with healthcare professionals and patients through various channels.Manage scheduling for visits and adherence to documentation timelines.COVID-19 Precaution(s) :

Licensed Vocational Nurse License in California.Preferred 1-2 years of experience in Home Health and Hospice.Current CPR Certification.IV Certification.Experience with wound care and / or wound vac preferred.Willingness to drive within 30-60 miles per day.Ability to pass drug and background screenings.Strong clinical and communication skills.Proficient computer skills.Reliable transportation and insurance.
