HonorVet Technologies is seeking a Social Work LMSW - Licensed Master Social Worker for a travel job in Kent, Washington.

Title : Licensed Masters Social Worker (LMSW)

Location : Kent, WA

Duration : 13 weeks

Shift : Day Shift, 5x8-Hour (08 : 00 - 17 : 00)

Job Responsibilities and Required / Preferred Skills :

Provide social work services to geriatric patients, including one-on-one and group counseling.

Conduct comprehensive social work assessments considering social, emotional, cognitive, and cultural factors.

Facilitate individualized, integrated care planning and access community and financial resources.

Perform social work interventions in various care settings including participant homes (moderate amount of fieldwork within South King County).

Coordinate and document care delivery and patient / caregiver education to support discharge planning and continuity of care.

Participate in care conferences and collaborate with interdisciplinary teams.

Required Experience & Qualifications :

Licensed Masters Social Worker with WA LICSW, WA LSWAIC, or WA LASW license required at time of submission.

Minimum 2 years of experience working with elderly and diverse populations.

BLS certification required.

Experience with EPIC charting system preferred.

Knowledge of adult protective services, guardianship, power of attorney, hospice, advance directives, and cultural issues.

Familiarity with common geriatric disease processes (CHF, COPD, dementia, diabetes, neurological and psychiatric disorders, stroke).

Strong case management and discharge planning skills, including knowledge of Medicaid and Medicare programs.

Preferred Skills :

Ability to coordinate services such as home care and equipment post-discharge.

Skilled in documentation, care coordination, and patient / caregiver education.

Comfortable working autonomously and scheduling home visits within work hours. Other Information :

No weekend or on-call requirements.

Float between facilities within the state as needed.
