Job DescriptionJob Description

The Transitions-Bronx Program is a specialized borough-wide prevention program for families challenged by substance use and / or mental health issues. The Case Planner provides home-based, intensive, strength-based, comprehensive services using the Solution-Based Casework (SBC) framework.

Reports To : Supervisor

Location : 2471 Morris Ave. Bronx, NY 10468 (Position is onsite. Hybrid schedule is at discretion based on needs of the program.)

Hours : 35 hours, non-exempt

Schedule : Monday - Friday; 9am-5pm with flexibility for family needs. Must be able to work at least 2 evenings per week to accommodate working families’ schedules
