Position Summary : Carolina Senior Care, a proud provider of PACE (Program of All-Inclusive Care for the Elderly) services, is seeking a dedicated and compassionate Physical Therapist to join our dynamic team in New Bern, NC. As a Physical Therapist, you will play a vital role in helping our participants maintain independence, dignity, and quality of life.
Essential Duties and Responsibilities :
- Conducts Assessments : Performs initial and annual physical therapy assessments for all participants, coordinating with the Interdisciplinary Team to develop and update comprehensive Plans of Care.
- Provides Treatment : Delivers physical therapy treatments, implements procedures to achieve goals, and grades activities. Confers with the Interdisciplinary Team on care issues.
- Evaluates and Recommends : Assesses participants' needs, follows up during treatment, and recommends solutions. Evaluates the need for adaptive equipment and ensures proper fit.
- Documentation and Reporting : Evaluates charts, documents progress, and maintains physical therapy notes according to charting requirements. Assists with data collection and prepares reports.
- Supervision and Training : Directs and supervises Physical Therapy Assistants, performs competencies, and provides ongoing education and clinical supervision. Trains staff on body mechanics and equipment use.
- Maintains Safety and Compliance : Ensures a safe working environment, follows policies and procedures, handles biohazard precautions, and participates in continuing education and quality improvement initiatives.
Job Requirements
Minimum Education :
Physical Therapy License in North Carolina
Minimum Requirements :
CPR Certification (Required)1 Year Working with Seniors (Preferred)Other Requirements :
Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.Must have a valid driver’s license, proof of insurance and have means of transportation.Benefits :
