- Reviews, processes, and modifies, as necessary, all medication orders to maintain a current and accurate medication profile.
- Supervises, directs and reviews all activities involved with the compounding, storing, inventory, labeling, recording, or preparing medications by pharmacy technicians.
- Monitors drug therapies to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic levels, drug interactions, and therapeutic duplication.
- Monitors the use of drugs for compliance with prescribing criteria, guidelines, formulary, and protocols established by Norwegian American Hospital.
- Detects, monitors, documents and reports adverse drug reactions and medication errors.
- Serves as a resource of drug information for all practitioners and patients (i.e. drug identification, side effects, adverse events, interactions, medication counseling, etc.).
- Participates in pharmacy quality assessment / performance improvement (QA / PI) projects and / or initiatives.
- Assures proper control, documentation, and record keeping of controlled substances.
- Ensures all state and federal regulatory requirements are maintain
Requirements :
Graduate of an ACPE accredited college of pharmacy, Pharm D preferred. Consideration may be given to graduates of foreign colleges of pharmacy who are appropriately licensed.
Illinois Pharmacist licensure required.
