Job Description

Job Title : Registered Nurse (RN) - Case Manager

Job Specialty : Care Coordination / Case Management - Utilization Review

Job Duration : 13 weeks

Shift : 8-hour days, every other weekend, first to float

Guaranteed Hours : 40 hours per week

Experience : Minimum of 2 years

License : Must possess a valid RN license.

Certifications : BLS

Must-Have : Prior experience with Dignity Health

Job Description :

• Perform case management and care coordination in all areas of the hospital.
• Handle both utilization review and discharge planning responsibilities.
• Engage with various patient demographics, including those dealing with all payers, the uninsured, and the homeless.
• Conduct initial assessments, IQ (Interqual), and discharge (DC) planning.
• Utilize Cerner for charting.
• Collaborate with hospitalists or intensivists who are in-house.
• Adapt to weekend staffing requirements; at least two weekends per month.
• Adhere to dress code requiring business casual attire.
• Participate in 5 days of care coordination new employee orientation.
• Ensure effective communication and teamwork within the hospital setting.
• Reprioritize tasks throughout the day as needed.
• Training provided in Interqual; both DC Planning and Utilization Management are essential.

Note : Holidays are not required, but willingness to work is appreciated if necessary. The hospital may cancel up to 4 shifts per 13-week assignment.
