As a Licensed Practical Nurse for Homeland HomeHealth, you would contribute to our community of caring by providing direct support to patients where they feel most comfortable, promoting and fostering their healing process.

Using compassion and skill, our nurses provide physician-ordered medical treatment to promote healing, independence, and the best possible quality of life. We believe in a multiple-disciplined approach to help patients in their healing journey, which includes wound care, cardiac care, palliative care, fall prevention, rehabilitation services, intravenous therapy, and other care services.
As Homeland HomeHealth's LPN you would provide skilled nursing care to the Home Health patient as directed by the supervising Registered Nurse (RN) and Director of Home Health, in compliance with Medicare Home Health Conditions of Participation, Pennsylvania licensure laws and agency policies and procedures. Provide timely documentation of patient services based on the patient's plan of care. Work closely with other members of the Home Health Interdisciplinary Team to ensure all patient and family care needs are being met.
Requirements include :
Graduate of an accredited practical nursing program.
Valid Pennsylvania Licensed Practical Nurse license.
