The Certified Registered Nurse Practitioner (CRNP) will provide comprehensive and compassionate care across the lifespan, with a focus on patients living with or at risk for HIV. The ideal candidate will work collaboratively with other medical and mental health providers, nurses, and support staff to deliver high-quality, compassionate patient-centered care.
We operate on a 35 hour work week (Monday to Friday 9AM - 4PM) and offer an excellent benefit package :

Responsibilities :
Deliver primary and specialized care for patients of all genders living with HIV, including diagnosis, ART initiation, monitoring, and adherence counselingProvide pre-exposure prophylaxis (PrEP) and post-exposure prophylaxis (PEP) as appropriate, STI screenings, and risk-reduction counselingCoordinate care and referrals for behavioral health, case management, housing, substance use services, and other social determinants of healthAdvocate for patient-centered care using trauma-informed, culturally competent approachesMonitor and manage co-occurring conditions, including hepatitis, and chronic diseasesPerform and interpret diagnostic tests such as Pap smears, STD screenings, complete blood counts (CBCs), electrocardiograms (EKGs) and radiographs (x-rays)Address gynecologic and reproductive health needs including contraception, prenatal / postnatal support, menopause management, and cervical cancer screening as neededEducate patients on health promotion, disease prevention, and reproductive healthRecommend interventions to modify behavior associated with health risksCollaborate with interdisciplinary teams for continuity of careConsult with or refer patients to appropriate specialists when conditions exceed the scope of practice or expertiseDevelop treatment plans based on scientific rationale, standards of care, and professional practice guidelinesPerform primary care procedures such as suturing, splinting, administering immunizations, taking cultures, and debriding wounds as needed and appropriatePerform routine or annual physical examinationsPrescribe medications based on efficacy, safety, and cost as legally authorized.Accurately document patient care using electronic medical records (EMR) in a timely mannerParticipate in quality improvement initiatives and continuing education.Keep abreast of regulatory processes and payer systems such as Medicare, Medicaid, managed care, and private sourcesMaintain departmental policies and procedures in areas such as safety and infection controlGreet and acknowledge patients according to agency standardsReceive, record and relay messages accurately, completely and legiblySpeak with others using clear, professional and polite languageDevelop and maintain positive working relationships with others, support the ACG team to reach common goalCoordinate and support the medical support staff by leading by exampleOther duties as assignedRequirements :
