Colorado Licensed Pediatric Nurse Manager

A company is looking for a Newborn / Pediatric Nurse Care Manager who is passionate about holistic healthcare for members.

Key Responsibilities :

Deliver coordinated, patient-centered virtual Care Management by telephone and video to improve members' health outcomes

Create impactful care plans with members and the care team, assisting members in achieving their goals

Help members navigate complex medical conditions, treatment pathways, benefits, and the healthcare system

Required Qualifications :

Bachelor of Science in Nursing (BSN)

Current CCM Certification or eligibility within 6 months of hire

5+ years of experience in nursing

2+ years of experience working in care management

Active Compact RN license in good standing with the nursing board of their state
