About The Position

Experience : Prior experience with assigned patient population required.
License : License not required but preferred.
Certifications : LCSW (Licensed Clinical Social Worker) or LPC (Licensed Practical Counselor) preferred.
Description :
Provides skilled interventions for patient and family crises.
Facilitates problem-solving and decision-making processes.
Engages in advocacy and facilitation for life-changing events, such as adoption, guardianship, abuse, placement, and bereavement.
Mediates risk factors and assists patients coping with disease or disability.
Addresses psychosocial and socioeconomic issues while adhering to departmental policies and procedures.
Receives referrals and assesses patients and families in a timely manner.
Integrates social work plans into overall patient care through interdisciplinary collaboration.
Provides comprehensive case management, discharge planning, continuing care services, advocacy, and clinical social work services.
Delivers crisis intervention and patient / family education using individual and family modalities.
Offers skilled services in areas related to abuse and neglect, advanced directives, and end-of-life issues.
Utilizes hospital and community resources and entitlements effectively.
Refers patients, families, and hospital staff to appropriate services to ensure continuity and quality of care.
Develops specialized knowledge of resources pertinent to specific patient populations.
Facilitates efforts to obtain insurance coverage for necessary services.
Documents care plans, staff interventions, and outcomes promptly and completely in the medical record.
Completes statistical reports as required by the department.
Collaborates with the interdisciplinary team to develop complex discharge plans and transitional planning.
Pursues professional development through supervision and attendance at relevant educational events.
Participates in community education and support group activities as approved.
Exhibits independent judgment in decision-making from diverse alternatives.
Coordinates activities of others but does not supervise.
Works directly with patients, customers, and healthcare professionals.
Frequently interacts with individuals at the Director level or above.
Computer literacy is a must.
Fluency in an additional language beyond English is preferred.
