WCS Healthcare is partnering with a reputable assisted living organization seeking a qualified RN to work a Monday - Friday 9am-5pm schedule as a Nurse Case Manager conducting assessments for Long Term Care services in Manhattan.

Job Function :

Day Shift.

9am - 5pm.

Collaborate with members and their families to maintain the most independent living situation possible.

Ensure consistent care across the healthcare continuum by assessing and closely monitoring members' needs and status.

Authorize covered services and coordinate care across different payers.

Communicate and collaborate with members, family members, caregivers, primary care practitioners, and the interdisciplinary team.

Job Requirements : NYS RN License.

Bachelor of Science in Nursing (preferred).

Associate’s Degree in Nursing required.

Minimum of 3 years of RN experience.

Bilingual in Spanish or Chinese (preferred).

Must have Care Management, UAS, or Managed Long-Term Care experience.

Field-based assessment experience.
