LPN Dementia Services Coordinator (Webster Campus)

St. Ann's Community is seeking an LPN Dementia services coordinator for assisted living memory care center Rainier Grove located on our Webster campus.

The primary purpose of your job is to supervise resident care, implementation of case management, supervise medication assistance, and overall care provided by LPNs, Resident Care Aides and other employees on the Memory Care Unit (Rainier Grove), and to assure that the resident care meets / exceeds the requirements as specified by the NYSDOH.
