Position Overview :
One Community Health is looking for a Salud Community Health Worker (CHW) to provide health-related education with our diabetes patients within the Salud program and provide administrative and rooming support in clinic. The Salud program brings critical education and resources to those living with diabetes and pre-diabetes within communities of the Columbia Gorge.
Responsibilities :
- Assist the provider / nurse with HbA1c screening, blood glucose tests, patient instruction and education. Provide basic information about the screening / tests.
- Screen for health status and social determinants of health during appointments with Provider / Nurse.
- Assist with external pharmacy coordination.
- Assist with ordering Continuous Glucose Monitors (CGMs) for patients.
- Asist patients with understanding and navigating with 340 B program.
- Assist with downloading and inputting of data into Epic from glucometers, CGMs and from patients' written Blood sugar logs.
- Provide basic diabetes education and information about follow-up care at the conclusion of patient visits.
- Assist with completion of forms and paperwork such as prior authorization and patient prescription forms
- Ability to assist with Outreach Salud events as needed and document after visit summary, and coordinate referral processes.
- Provide basic patient education and information about follow-up care at the conclusion of patient visits.
- Handle patient communication relating to prescription renewals; current illnesses; chronic conditions; health-related information and education. Obtain information from outside agencies related to test orders, appointment times, etc.
- Respond to telephone calls and EHR messages according to established standards and time frames. Document all pertinent information and actions in the patient record.
- Maintain strict confidence of all protected medical information and One Community Health (OCH) operations information gained in the performance of duties including at assigned outreach sites. Follow proper HIPPA and OCH policy protocol.
- Work as a collaborative and positive team member to provide direct and indirect support of other teams to accomplish daily workflows to meet patient care needs.
- Communicate professionally and effectively with team members to facilitate continuity of care for patients.
- Assist with patient triage by facilitating communication between licensed medical providers and licensed nursing staff.
- Understand and support referral process and schedule patients.
- Complete documentation including medical records, logs, and reports as assigned.

Hourly position$20.00 / hr pre-certification
$25.00 / hr with Community Health Worker certificationBilingual Differential : Additionally, we offer $0.75 per hour as compensation for bilingual proficiency in both English and Spanish.
