Act as a care-coordinator and educator

The Opportunity :

• You will work closely with clinic providers on a one-on-one basis.
• Assist and support clinic providers in delivering high-quality patient care through tasks such as rooming, phlebotomy, injections, and patient communication and education.
• Triage incoming phone calls as needed to determine the urgency of requests.
• Develop and participate in programs aimed at enhancing the health of this population.
• Utilization of the Epic Electronic Medical Record system.
