Social Worker (New Grads Welcome)

Evergreen Commons Rehabilitation & Nursing Center is looking for a full time Social Worker for our long term care units.

Duties :

Organizes and develops a plan of care based on their individualized needs of the residents. Asseses regularly to create goals and modify as necessary.

Assists the residents, staff, and families with issues that impact or prevents adjustment or any other aspect of the residents quality of life.

Consults with other members of the interdisciplinary team to provide adequate services that enhance the social and emotional well-being of our residents

Consults with outside agencies if applicable. Responds to referrals and are initiated timely.

Attends meetings as assigned

Responsible for providing services to families where problems may interfere with resident's plan of care

Complete documentation and assessments as required

Requirements

Bachelors Degree and at least one year experience in long term care preferred but not required

Organization and attention to detail

Must be able to communicate and interact with all members of the health care team
