Job description

Description : SUMMARY :

The Disease Intervention Program Coordinator will be responsible for leading the day-to-day activities of the Substance Abuse and Mental Health Services Administration Minority AIDS Initiative (SAMHSA MAI) project. The position will actively participate in the development and implementation of effective and quality rapid screening and linkage to care services. The position will ensure that services provided are within the guidelines of agency mission and project requirements. This position is responsible for maintaining project compliance and all reporting as required by project guidelines and funding requirements.

REPORTING :

Reports to the MAT & MAI Program Director

POSITIONS SUPERVISED :

Disease Intervention Specialists (DIS) (3-4 team members)

DUTIES AND RESPONSIBILITIES :

  • Provide supervision for all program activities including HIV, Hepatitis C, and Syphilis rapid testing, linkages to treatment, prevention and harm reduction education, outreach and other direct support services.
  • Provide formal, individual supervision and support for assigned program staff including access to professional development, job orientation and training, that will enhance the ethical, confidential and respectful delivery of services to clients.
  • Lead the Disease Intervention team in the provision of education, case management and prevention services for high-risk HIV-negative clients.
  • Ensure the provision of evidence informed HIV / STI Prevention Education.
  • Facilitate weekly case review meetings with program staff and managers.
  • Assign caseload to MAI staff, monitoring case load size.
  • Conduct screening interviews for patients and coordinate care plans with the medical staff.
  • Maintain complete and accurate documentation, including maintaining clear and precise data and case notes and reporting reactive results to the state as required.
  • Ensure timely collection and collation of statistical information for reporting.
  • Coordinate with Program Director to ensure collection of all client reporting as required by the funder, along with other reporting requirements.
  • Perform other duties as assigned.

Requirements :

Skills / Knowledge / Abilities :

1. Excellent written and verbal communication skills with the ability to effectively communicate with diverse populations.

2. Cultural and diversity understanding and awareness

3. Interpersonal skills that promote and encourage teamwork

4. Advanced knowledge of HIPAA and confidentiality

5. Advanced knowledge of behaviors and emotions related to addiction

6. Advanced knowledge of community and referral resources for clients diverse needs

7. Skills in documenting effective case management notes

Education and Experience :

1. Bachelors degree (required), Master's degree in public health or related field (preferred)

2. At least 2 years experience in grant management and implementation. (required)

3. Experience with supervising staff and leading a team (preferred)

4. Experience working with vulnerable populations. (required)

5. Experience gathering and reporting on program data. (required)

6. Valid NM drivers license [at least 3 years] and clean driving record (required)
