As a Home Care RN, you will : - Deliver exceptional nursing care to clients in their homes, ensuring they receive the highest standard of medical attention and support. - Collaborate closely with a multidisciplinary team, including therapists, social workers, and other healthcare professionals, to provide comprehensive care tailored to each client's needs. - Follow physician's orders and work under the guidance and supervision of a Clinical Supervisor to ensure all medical protocols and care plans are meticulously followed. - Build meaningful relationships with clients and their families, offering not only medical care but also emotional support and reassurance during their health journeys. Responsibilities : Skilled Nursing Care and Comprehensive Assessment Teaching, Supervision, and Counseling Clients and Caregivers Interdisciplinary collaboration Problem solving and critical thinking skills Monitors patient progress to ensure accomplishment of patient goals Oasis documentation Provides appropriate care for a range of physician-ordered treatments (PleurX Drain, Finger stick Coagulation checks, wound care, wound vacs, IVs, phlebotomy, Trachs, Peg Tube, Ostomies, Telehealth)

Qualifications : Minimum of one (1) years recent experience as a professional nurse in Home Health. Must complete Point of Care Testing training as part of Department Orientation. Competencies Skills : Essential :

• Experience In Hospital Or Acute Care Setting
• Clear Communication Skills Both Written And Verbal
• Able To Keep Confidential Information Regarding Patients, Team Members
• Able To Withstand Crisis Situations
• Has Skills To Provides Customer Service To Patients, Team Members And Visitors
• Proof Of Current / Valid Car Insurance
• Knowledge And Experience With Electronic Health Records
• Valid DL With No More Than 3 Moving Violations Credentials : Essential :
• RN - Registered Nurse
• BLS - Basic Life Saving certification Entry : USD $36.20 / Hr.
