As a Primary Care Physician, you will provide high-quality, outpatient care in a value-based care model, focusing on building long-term relationships with patients while driving positive health outcomes.

Practice Highlights :

100% outpatient primary care

Patient population : seniors with 3–5 chronic / complex conditions (e.g., heart disease, diabetes, hypertension)

Average of 10–12 patients per day

Patient panel capped at ~650

Dedicated care team including MA, scheduler, referral specialist, and full leadership support
