Monday - Friday, 8 : 00am-4 : 30pm or 8 : 30am-5 : 00pm with occasional weekends

$40- $43 / hour

LIFE (Living Independence for the Elderly) is a Medicare / Medicaid program that helps people meet their health care needs in the community instead of going to a nursing home or other care facility. Job Description

• Participates as an active member of the interdisciplinary team, determines nursing goals of the care plan, and monitors the participant's progress toward goals.
• Provides skilled nursing care and prescribed treatments to participants in their home and in clinic.
• Directs and supervises the provision of quality para-professional clinical care to participants in their home.
• Conducts customer service home visit assessments of each new participant within the first week of program enrollment and performs re-assessments and evaluations of each participant.
• Assist with referrals to other services when participant conditions or situations require the service of other professional disciplines or the products or services of outside companies or agencies.
