BETHESDA

NURSING ASSISTANT / RESIDENT ASSISTANT

Job Summary : Provides quality, efficient and loving care to each resident in a personalized way. Assists in the common goal of maintaining the well being of each resident by working as a primary caregiver. Meets the basic needs of assigned residents by supervising or assisting residents with ADLs.

Job Relationships :

A. Reports to : Licensed Practical Nurse, Registered Nurse, and Director of Nursing.

B. Interrelationships with : Other Nursing Assistants, Transportation Aides, Rehabilitation Aides.

C. Internal Contacts : Activities, Housekeeping, Dietary, and Maintenance personnel.

D. External Contacts : Visitors, family and friends or residents, and volunteers.

Hours of Work : Shift work will vary depending on shift and floor

Day shift – 6 : 00 AM – 2 : 30 PM

Evening shift – 2 : 00 PM – 10 : 30 PM

Night shift – 10 : 00PM – 6 : 30 AM

Scheduled to work every other weekend, including holiday rotation.

Dress Code : Uniform projecting a professional image with identification name badge.

Essential Requirement of Work : (Minimum qualifications necessary to function at full productivity).

A. Training : Successful completion of an approved competency evaluation or an approved nursing assistant training program and competency evaluation within four months from the date of employment.

B. In-serve Education : No less than 12-hours per year to ensure continued competence and address any areas of weakness as determined by performance reviews.

C. Experience : Previous health care or geriatric healthcare desirable.

D. Knowledge, Skills, and Ability for Full Performance :

a. Ability and desire to follow all safety procedures.

b. Ability to read and write in English.

c. Ability to foster independence, self-respect, and self-worth in each client by consistent understanding and kindness.

d. Use of equipment including tubs, lifts, splints, but not limited to, and ability to take vital signs.

e. Knowledge of basic health and identifying changes in residents.

f. Ability to successfully complete tasks on time and to be of assistance to others.

g. Knowledge and appreciation of all rules and guidelines for resident care such as transfers and cleaning procedures.

h. Use of transfer devices, safety procedures, and proper lifting techniques.

i. Ability to use a computer.

E. Personal :

a. Ability to perform well with co-workers in a teamwork setting.

b. Respect for others, residents, and staff alike.

c. Ability to communicate.

i. Ability to interpret or explain certain procedures or activities, to enlist and maintain cooperation from residents.

d. Ability to read, understand, and follow directions.

e. Genuine interest and willingness to work with cognitively impaired and / or geriatric aged clients.

f. Time management skills.

Physical Demands of the Job :

1. Frequent bending and stooping

2. Standing for prolonged periods of time

3. Patient handlers must sign & comply with the Skilled Nursing Facility Safe Patient Handling Policy.

1. Working in an environment with many residents who exhibit a wide range of emotions and needs including occasionally aggressive residents.

11. Respecting resident’s rights leads to further respect toward the worker.

12. Cleaning resident’s rooms by putting objects in appropriate places will help others to find those items more quickly and more conveniently.

Principle Job Responsibilities :

Responsibility : Provide direct resident cares to assure resident needs are being met as directed on the care plans.

Tasks :

1. Communicate with resident, explaining, instructing, listening and directing.

2. Provide AM / PM cares to assigned residents, according to resident plan of care. Cares include either performing these duties or assuring that the resident has done them :

a. Partial bath in AM and at hour of sleep, including washing face, hands, back and peri care.

b. Oral cares / denture care.

c. Hair care - redo hair when needed, especially after naps.

d. Dressing / undressing.

e. Toileting.

f. Full baths as assigned, set women’s hair as needed. Clip toenails and fingernails on bath day if resident is not diabetic. Have nurse do diabetic’s nails.

g. R.O.M. per plan of care.

3. On the night shift, do rounds on assigned resident; toileting, turning, repositioning and providing cares as needed.

4. Answer lights promptly and pleasantly and meet the needs of the residents.

5. Assist the residents with bowel and / or bladder cares and maintain cleanliness. Residents with no control of bowel and bladder must be checked minimally every two hours.

6. Take TPR, blood pressures and weights when requested and on every bath day.

7. See that residents are properly, comfortably and attractively clothed at all times, changed as necessary and assist with clothing changes as needed.

8. Provide privacy and assistance for residents and visitors.

9. Assist residents with the care of personal belongings and marking of the same, unmarked clothing to be brought to laundry department for labeling.

10. Participate in the rehabilitation and restorative efforts of the residents including ambulating, repositioning every two hours on residents unable to move themselves, and A.R.O.M. and P.R.O.M.

11. Assist in obtaining urine and stool specimens, as ordered.

12. Assist nurses as necessary, performing any other duties as assigned.

Responsibility : Provides assistance with meals to assure that resident’s nutrition and hydration needs are being met.

Tasks :

1. Assist residents to and from meals as needed.

2. Pass clothing protectors as requested or needed.

3. Do not uncover food if resident not ready for meal, reheat when needed.

4. Feed residents requiring assistance.

5. Monitor dining room during meal times.

6. Offer residents second choice if they dislike meal.

7. Document meal monitors for all residents.

8. Document accurate intake records on residents as ordered.

9. Residents to eat in their room only by order of the charge nurse.

10. Pass and assist residents with nourishments sent up between meals.

Responsibility : Maintain ward order to keep unit neat, orderly and safe.

Tasks :

1. Night shift place bed packs in resident’s rooms on their bath day.

2. Day shift strip bed when they get resident up, housekeeping wash bed and nursing assistants remake bed with clean linen provided in bath pack.

3. Pass fresh ice water to each resident every shift.

4. Re-stock drawers when supplies are low.

5. Make residents bed in AM after getting resident up, replace any soiled linens. Rinse out grossly soiled linens immediately, before sending to laundry.

6. Empty laundry barrels by 8 : 00 AM on day shift, end of shift on PM shift and 5 : 00 AM on night shift.

7. Keep hallways free of clutter – all supplies and equipment to be kept on one side of hallway.

8. Maintain proper storage of supplies and chemicals. Any cleaning product or hygiene item with a warning label stating : potential harm can occur if used improperly or ingested, must be stored in a locked cabinet.

Responsibility : Receive and give information regarding residents.

Tasks :

1. Read 24-hour report sheets prior to starting your workday.

2. Perform daily documentation as indicated on KIOSK.

a. Vital signs as assigned.

b. Bowel and bladder sheets.

c. Quantitative behavior documentation.

d. Meal monitors.

3. Give verbal report on status of assigned residents to nurse prior to end of shift. Any significant changes or concerns about residents should be reported to the nurse promptly. Do not wait until the end of shift to report these things.

4. Answer telephone and intercom politely and professionally and relay messages on a timely basis.

Responsibility : Provide a safe environment for residents and staff.

Tasks :

1. Be familiar with and obey all laws regarding residents rights and vulnerability, including reporting any incidents of known or suspected resident abuse or neglect

2. Report resident and staff incidents promptly to supervisor.

3. Utilize power lift technique with all work.

4. Use transfer belt for resident transfers and ambulation.

5. Clean up spills immediately.

6. Clean and disinfect tubs after each use and sanitize at end of shift.

7. Provide aseptic techniques to prevent spread of disease and infection. Wash hands promptly after providing any direct resident cares.

Professional and Personal Growth :

1. Attend in-services – 12-hours required per year.

2. Attend floor meetings.

3. Participate in employee involvement groups, as requested.

4. Use available resource materials.

5. Maintain a cooperative attitude towards assisting co-workers and other departments.

6. Accept constructive criticism and suggestions gracefully.

7. Assist in orientation of new personnel, as assigned.

8. Maintain personal health, to minimize absence from work.

9. Report for and leave the work area on a timely basis.

10. Report any absences on a timely basis.

11. Participate in annual performance evaluation process.

12. Maintain a professional conduct and pleasant, friendly attitude towards staff, residents and visitors.

13. Be willing to learn and change to meet needs that arise.

14. Provide support to other staff, recognizing the extreme stresses of this job.
