Our Hospice Aide (CNA / LNA) Will :
Work independently and take initiative to manage a caseload.Have autonomy and flexibility in managing their schedule.Utilize varying skills as a member of the clinical team as well as the community relations team.Develop meaningful, effective, and professional relationships with communities, families, and patients.Provide patients assistance with personal care including, but not limited to, baths, skin care, back rubs, oral hygiene, hair care, dressing, meal assistance, and light housekeeping.Assist in supporting the patient's psychosocial wellbeing through companionship.Assess and report on patient's condition and significant changes to the RN Care Manager.Assist with community relations events and activities including Vitals Clinics, seasonal activities, education sessions, meet and greets, etc.Requirements
Our Hospice Aide (CNA / LNA) Has :
An active, unencumbered, state-required CNA certification.1+ years of experience working as a CNA in home health or hospice.The ability to work with little direct supervision.The ability to interact professionally with patients, families, and fellow team members.An active Driver's License with a reliable vehicle, insurance, and an acceptable driving record.The ability to assist residents with limited mobility, lift / push / pull up to 75 lbs.The ability to provide a calm manner when in a patient's home.
