Our Hospice Aide (CNA / LNA) Will :

  • Work independently and take initiative to manage a caseload.
  • Have autonomy and flexibility in managing their schedule.
  • Utilize varying skills as a member of the clinical team as well as the community relations team.
  • Develop meaningful, effective, and professional relationships with communities, families, and patients.
  • Provide patients assistance with personal care including, but not limited to, baths, skin care, back rubs, oral hygiene, hair care, dressing, meal assistance, and light housekeeping.
  • Assist in supporting the patient's psychosocial wellbeing through companionship.
  • Assess and report on patient's condition and significant changes to the RN Care Manager.
  • Assist with community relations events and activities including Vitals Clinics, seasonal activities, education sessions, meet and greets, etc.

    • Requirements

    Our Hospice Aide (CNA / LNA) Has :

  • An active, unencumbered, state-required CNA certification.
  • 1+ years of experience working as a CNA in home health or hospice.
  • The ability to work with little direct supervision.
  • The ability to interact professionally with patients, families, and fellow team members.
  • An active Driver's License with a reliable vehicle, insurance, and an acceptable driving record.
  • The ability to assist residents with limited mobility, lift / push / pull up to 75 lbs.
  • The ability to provide a calm manner when in a patient's home.
