Role Overview :

As a Geriatrician (Home-Visiting Physician) , you will provide direct medical care to elderly patients in their homes. You will diagnose and manage chronic conditions, collaborate with interdisciplinary teams, and guide families on complex healthcare decisions, always prioritizing patient well-being and dignity.

Key Responsibilities :

• Conduct in-depth medical evaluations and develop personalized care plans.
• Manage chronic conditions, adjust medications, and address acute concerns.
• Collaborate with NPs, PAs, nurses, therapists, and social workers.
• Educate patients and caregivers on disease prevention and health maintenance.
• Document patient encounters and maintain compliance with regulatory standards.
