Job description

Pharmacist - Long-Term Care

Location : Monsey, NY (On-site)

Compensation : $125,000-$150,000 base salary (negotiable) + full benefits and additional perks

Our client, a rapidly growing and well-respected pharmacy organization in the Monsey area, is seeking a Pharmacist to join their expanding team. The company partners with Long-Term Care (LTC) facilities to provide comprehensive pharmacy management services, ensuring residents receive the highest quality of care. Key Responsibilities

  • Partner with LTC facilities to manage Prior Authorizations , Formulary Exceptions , Step Therapy , and Quality Care Dosing programs.
  • Provide clinical expertise and resolve questions related to formulary and non-formulary medications , ensuring compliance with policies and procedures.
  • Collaborate closely with physicians, nurses, and facility staff to optimize medication management and patient outcomes.
  • Conduct clinical chart reviews on a scheduled basis and document all interventions to enhance pharmacotherapy.
  • Support continuous quality improvement through proactive problem-solving and clinical diligence.

Qualifications

  • Licensed Pharmacist with prior experience in a Long-Term Care pharmacy environment (required).
  • Strong understanding of therapeutic interchanges and medication management for LTC populations.
  • Excellent written, verbal, and interpersonal communication skills.
  • Highly organized with the ability to prioritize multiple projects and work independently.
  • Demonstrated analytical and problem-solving skills, with a commitment to teamwork and collaboration.
