Job description
A company is looking for a Medical Assistant (REMOTE).
Key Responsibilities :
Conduct comprehensive patient assessments and identify barriers to effective disease management
Assist in developing and implementing individualized care plans in collaboration with a multidisciplinary team
Educate patients on managing chronic diseases and facilitate communication among healthcare team members
Qualifications and Requirements :
Certified Medical Assistant (MA) credential required
At least 2 years of experience in patient care or chronic disease management
Proficiency in electronic health records (EHR) systems
Reliable internet connection and a HIPAA-compliant workspace
