Key Responsibilities:

• Provide comprehensive wound care to patients in various settings, including homes and clinics, ensuring optimal healing and comfort.
• Assess, evaluate, and manage patients' wound care needs, developing personalized treatment plans.
• Collaborate with a multidisciplinary team to coordinate patient care and ensure the best outcomes.
• Educate patients and their families on wound care management and prevention strategies.
• Maintain accurate and timely documentation of patient care and progress.

Qualifications:

• Must have a Woundcare certification
• Current and unrestricted Nurse Practitioner license in the state of California.
• Certification in wound care or relevant experience is preferred.
• Excellent clinical skills and the ability to work independently.
• Strong communication and interpersonal skills.
• Valid driver’s license and reliable transportation for travel within the Thousand Oaks area.
