RN After Hours Care Manager

A company is looking for a Telephonic After Hours Nurse Care Manager (WEEKENDS).

Key Responsibilities

Handle inbound and outbound communications with patients regarding symptoms and care concerns

Consult and coordinate with team members to assess and implement patient care plans

Provide health information and advice while documenting member data in the medical record system

Required Qualifications

Registered Nurse (RN) with current compact licensure in assigned state(s)

Bachelor's degree or higher

Minimum of 3+ years of recent clinical experience in a relevant setting

Experience in disease management, case management, or utilization review

Strong computer skills and ability to work in a remote environment
