Job Title : Registered Nurse I

Job Specialty : Home Health

Shift : Day 5x8-Hour (08 : 00 - 17 : 00), Monday to Friday

Guaranteed Hours : Not applicable

Experience : Minimum of 1 full year Adult Home Health experience or 1 full year Hospice experience required

License : Must have license in hand (no pending licenses accepted)

Certifications : CPR, BLS

Must-Have : Proficiency in HomeCare HomeBase and OASIS used within the last 6 months or completion of a retraining course during compliance

Job Description :

• Provides, coordinates, and directs the provision of home nursing care according to physician's orders based on agency policies and procedures.
• Responsible directly to the Clinical Manager and directly supervises and evaluates Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.
• Required to perform home visits with reliable transportation, a current driver's license, and agency-required liability insurance.
• Average number of patients per day is 5-8, equating to roughly 6-8 points per day.
• Types of cases include Start of Care, Wound Care, and Follow-Up Visits.
• Must be willing to travel up to 75 miles from the home clinic, with mileage reimbursed at the IRS standard rate for travel from patient home to patient home.
• No guarantee of hours; clinicians may float to nearby clinics if necessary to meet financial needs.
• Clinicians must complete all documentation within 24 hours of the appointment with the patient.
• Local candidates are accepted at the same rate.
• Routine weekly productivity expectation of 30 units, correlating with 40 hours per week.
• No weekend rotations or on-call requirements for the standard schedule; however, this may vary with location and profession.
• Open to traveling clinicians up to 75 miles from the assigned care center.
• Clinicians cannot currently be direct employees of specific healthcare providers if applying as a traveling nurse.
• Candidates must accept all patients on their schedule regardless of distance from the previous appointment.
