Description :
We are seeking a Registered Nurse specializing in Case Management.
The position will involve managing a caseload of 14-16 patients depending on the unit assigned.
The candidate must have experience in adult medical / surgical, cardiac, or orthopedic care.
Care management skills such as assessment, care coordination, and discharge planning are essential.
The RN will work routinely with diverse populations, including the homeless.
The department culture is supportive and emphasizes teamwork.
The management team is communicative and provides support through changes.
The facility utilizes EPIC as the electronic medical record system.
Travelers will be expected to work all holidays unless previously approved.
Shifts will typically follow a schedule of five 8-hour shifts each week.
Scrub colors are navy blue for RN positions.
