Home Health Agency looking to hire full time Patient Services Specialist / Scheduler

Preferred Transitions Home Health Care has experienced a growth in demand, as we are one of the highest rated agencies in the entire state. The latest CMS ratings released in January 2025 have Preferred Transitions scoring a perfect 5 Star Rating in BOTH Patient Satisfaction Survey and Patient Outcome. Therefore, we are looking to fill the position of Nursing Supervisor.

DUTIES OF POSITION :

• Is responsible for the coordination of patient visits, maintenance and upkeep of scheduling.
• Staffing coordination duties include the accurate and timely communication of scheduling changes between, patient, office and field staff.

POSITION RESPONSIBILITIES :

Update schedules regularly.

Must be able to cover a weekend if needed.

JOB CONDITIONS :

Position is stressful in terms of meeting deadlines.

EQUIPMENT OPERTATION :

QUALIFICATIONS : 1. RN license

2. Preferably someone with a Home Health background.

3. Should be skilled organizer able to manage office files, log books and staff schedules.

4. Must possess light secretarial skill and have a polite telephone manner.

5. Must have a criminal background check.
