Horizon San Gabriel Valley is seeking a detail-oriented and compassionate Medical Care Coordinator to support our patients by ensuring coordinated, efficient, and high-quality healthcare services. This role is ideal for someone who excels at communication, organization, and care coordination in a fast-paced healthcare environment.
The Medical Care Coordinator plays a crucial role in facilitating communication and coordination between healthcare providers, patients, and other stakeholders. This position manages the administrative and logistical aspects of patient care while organizing, implementing, and monitoring healthcare plans to help optimize patient outcomes.
Coordinate communication between patients, healthcare providers, and internal teams
Assist with organizing and implementing individualized healthcare plans
Monitor patient progress and follow up on care needs and services
Manage administrative aspects of patient care, including documentation and scheduling
Ensure timely coordination of appointments, referrals, and services
Maintain accurate, confidential patient records
Collaborate with interdisciplinary teams to support continuity of care
Identify and address barriers to care in a timely manner
Experience in healthcare coordination, case management, medical administration, or related role preferred
Strong organizational and multitasking skills
Excellent communication and interpersonal abilities
Ability to work independently and collaboratively
Detail-oriented with strong documentation skills
Experience working with vulnerable populations a plus
Knowledge of healthcare systems, care planning, or social services preferred
