QMHP-A needed to provide mental health skill building services.  Must be self-motivated and responsible.  Experience is strongly desired. Must have own transportation and be able to work independently. Must be able to provide support and reinforcement of functional skills and behavior related to the individuals' health and safety, ADL's and use of community resources, assistance with medication management, and monitoring health, nutrition, and physical condition.
