What You'll Do :

Provide skilled nursing in private homes

Administer medications, treatments, and injections

Monitor vital signs, report changes, and update care plans

Deliver patient education to clients and families

Perform advanced nursing tasks (e.g., wound care, G-tube, trach, catheter)

Maintain accurate and timely documentation

Represent Affirmed with white-glove professionalism and heart

Who We're Looking For :

Active LPN license in Connecticut

1+ year of experience (home care, hospice, rehab, or facility)

Strong clinical skills and confidence in independent practice

Reliable transportation and valid driver's license

Professional communication and bedside manner

CPR / BLS certification required

Familiarity with electronic charting preferred
