The ideal candidate for this role, Full Time Nurse Practitioner - Family Medicine, will be passionate about providing care and educating the patients in this community! - Delivery of a variety of healthcare services, including preventative, chronic and acute care for a wide variety of patients (infant to geriatric)
- Perform diagnosis and treatment of illnesses, carry out health-risk assessments, screening tests, and counseling to individuals on the Observation Unit
- Ensure participation in continuing education and training to keep abreast of current industry developments
- Provide quality care to patients by following the most current evidence-based guidelines and procedure of practice
- Maintain accurate patient records and participate in quality assurance audits
- Provides medical and mental health education services to patients, their families, and care-takers
- Obtains detailed medical history and perform physical examination on assigned patients
- Diagnoses and prescribes treatments, both pharmacological and nonpharmacological
- Develops and implements patient management plans, records progress notes, and assists in the provision of continuity of care
- Counsels and instructs patients regarding compliance with prescribed therapeutic regimens and health maintenance
- Collaborate with supervising physician and other members of the healthcare team in care coordination / clinical meetings
