Minimum Qualifications :

Bachelor's degree with progressive experience or a Master's degree in Physical Therapy

Current license to practice as a Physical Therapist in Ohio

Working knowledge of Conditions of Participation and home healthcare experience preferred

Valid driver's license and reliable transportation
