Description : POSITION SUMMARY :

The Caseworker Aide supports the Social Services Department by providing essential case management support. Responsibilities include delivering home-based intervention services and prevention activities, along with proficiency in outreach, referrals, networking, and recordkeeping. Prioritizing organizational efficiency and confidentiality, tasks encompass appointment scheduling, file management, and compiling client histories. Collaborating with various agencies, the aide assists in planning preventive initiatives and ensures clients access necessary resources and support services.

DUTIES / RESPONSIBILITIES :

• Assist caseworkers in conducting intake interviews with clients to assess needs and develop action plans.
• Maintain accurate and up-to-date client records, ensuring compliance with confidentiality policies and procedures.
• Coordinate client appointments and follow-up meetings, ensuring timely communication and scheduling.
• Provide administrative support, including drafting correspondence, completing forms, and organizing documentation.
• Conduct research to identify community resources and support services available to clients.
• Participate in team meetings and case conferences to discuss client progress and address complex cases.
• Provide crisis intervention and support to clients experiencing emergencies or challenging situations.
• Assist with outreach and educational activities to raise awareness of available social services and resources.
• Monitor and report on program outcomes and effectiveness, contributing to continuous improvement efforts.
• Collaborate with community partners and agencies to coordinate services and referrals for clients.
• Support caseworkers in conducting home visits and assessing living conditions and family dynamics.
• Maintain professional development by attending training sessions and staying informed about best practices in social services.
• Other duties as assigned.
