Manager of Care Management

A company is looking for a Manager, Care Management.

Key Responsibilities

Manage and develop a team of Team Leads and Care Coordinators, ensuring high-quality patient-centric care

Oversee operational effectiveness and drive performance metrics to meet service goals

Lead process improvement initiatives and manage change within the care management workflow

Required Qualifications

3-5 years of direct leadership experience in care management

Experience leading remote teams through growth and change

3-5 years of experience in integrated Care Management or telehealth, preferably in Chronic Care Management

Experience in a healthcare setting, such as acute care or managed care

Active healthcare license (LVN, RN, LMSW, LCSW preferred) and a Bachelor's Degree in Nursing or a Master's in Social Work preferred
