Job description

Overview

Public health nurses promote and cultivate systems-level improvements in their community, with a focus on primary prevention, through adherence to the principles and standards of Public Health Nursing, and direct application of the Public Health Nurse Competencies. The role of public health nurse presents a unique opportunity to have broad, meaningful impact on the health and well-being of specific populations and ensure health equity. This position will work with the community partners and systems (direct health care, schools, other state agencies) involved in the care of families and children in Bennington County to foster coordination and collaboration and promote best population outcomes. This is not a direct care / clinical care or individual case management position. This position is a hybrid position.

The Family and Child Health Coordinator focus on eliminating health disparities and increasing health equity for people of reproductive age, people who are pregnant and post-partum, infants, children, and families in Bennington County. This important work, with a focus on prevention across the lifespan including preconception, achieves long term, sustainable impacts on the health of this population by developing and maintaining relationships with the local systems of care for children and families. Although there is some interaction with direct service providers in the community, the FCH Coordinator primarily provides a public health perspective to direct service work and encourages coordination of services. The role of School Liaison involves close communication and collaboration with school personnel (e.g., nurses, administrators, counselors, teachers) to help schools address the health and wellness needs of their students and achieve optimal physical, social, and emotional health within their school communities. The liaison promotes access to care among school-aged children, provides support and technical assistance to school nurses, and provides guidance and technical assistance to school wellness committees in the planning and proposed implementation of goals and objectives and proposed reinvestment of Medicaid funding.

TELEWORK :

State of Vermont telework policy requires at least three days a week working in the office. The Department of Health supports a hybrid work environment which combines both in person and remote work. Upon hire, employees work with their supervisor to create a schedule that aligns with the business needs of the unit and complies with State of Vermont policy. AHS BACKGROUND CHECKS : Candidates must pass any level of background investigation applicable to the position. Environmental Factors

Duties primarily performed in a standard office setting. Work may be performed in clinic settings and possibly client homes. Exposure to communicable diseases and other stressful situations may occur. Private means of transportation is required. May be required to work evenings, weekends and 12 hour shifts.

Minimum Qualifications

Associate's degree in nursing AND two (2) years or more of professional nursing experience.

OR

Bachelor of Science in Nursing (BSN).

OR

College senior in the final semester of a bachelor's degree in nursing (BSN) program. (Graduation from the BSN program must occur prior to hire and continued employment will be contingent on the employee passing the Nursing Licensure Exam (NCLEX).

Possession of, or eligibility for, licensure as a Registered Nurse (RN) in VermontOR eligible to practice in the state of Vermont via a multi-state license.

NOTE : Must maintain Vermont licensure or multi-state license as a Registered Nurse as a condition of employment.

Preferred Qualifications

Bachelor of Science in Nursing (BSN).
