Lifesong Hospice is a subsidiary of Country Meadows Retirement Communities. Lifesong is now hiring a Hospice Nurse Practitioner to join our dynamic team!

The Hospice CRNP works in collaboration with the Hospice Medical Director, Hospice Physician(s), Clinical Team, and Attending Physician to provide Hospice and Palliative care services to patients and families in a variety of patient care settings.

The Hospice CNRP will participate in the development and implementation of medical policies, procedures, and protocols; comply with state and national regulations; represent Lifesong Hospice and Palliative Care (LHPC) in interactions with physicians, hospital staff, facility staff, and others in the community.
