Job SummaryGeisinger is seeking an
experienced Nurse Practitioner or Physician Assistant for a per
diem position in various skilled nursing facilities as
needed.
Job Duties
This position requires an
experienced advanced practitioner, prior experience working in a
skilled nursing facility (SNF) is helpful but not required.
The SNF advanced practitioner (AP) provides direct
(hands-on) care in nursing homes / skilled facilities with a goal to
improve quality and prevent unnecessary re-admissions to the acute
care
setting.
Overview : Hours :
Monday – Friday 8 a –
4 : 30p
Responsibilities
include : Communicate with
SNF staff and review patient reports
Treat
acute needs, admissions, discharges,
etc.
Manage complex and non-complex
patients
Average 12 patients a
day

Family Nurse Practitioner, Adult – Geriatric Nurse
Practitioner or Physician Assistant with advanced practice
experience in a skilled nursing facility or long-term care facility
is preferred.
