Rehabilitation Manager – Managed Long-Term Care (MLTC)

Overview :
We are seeking a Rehabilitation Manager to support the clinical oversight and delivery of rehabilitation services within Managed Long-Term Care (MLTC), MAP, and ESNP programs. This role focuses on ensuring members receive appropriate care and equipment to support independence and quality of life at home.
Responsibilities :
- Review rehabilitation documentation from external providers to ensure services are clinically appropriate.
- Evaluate and process requests for Durable Medical Equipment (DME) for home use.
- Provide education and guidance to nursing leadership regarding rehabilitation services and care planning.
- Perform home and field assessments as needed (including evaluation for complex DME and home safety).
- Maintain compliance with New York State Department of Health regulations.
