• ? Role Overview

The Social Service Assistant plays a vital role in supporting residents and families through the care process. This includes conducting assessments, facilitating transitions of care, supporting healthcare directives, and collaborating with the interdisciplinary team.

• ? Key Responsibilities

Assess cognitive capacity and mood using standard tools.

Facilitate healthcare directives and end-of-life care discussions.

Support discharge planning in collaboration with the healthcare team.

Provide culturally sensitive training and education for staff.

Participate in new employee orientation (resident rights, abuse / neglect, behaviors, culture change).

Serve as part of the leadership team and participate in Manager on Duty rotation.

Stay current with all state and federal regulations.

Other duties as assigned.

• ? Qualifications

Bachelor’s degree in Social Work (required)

LSW (preferred)

3+ years of long-term care or healthcare experience (preferred)

Proficiency in Microsoft Office (Excel & Word)

Strong teamwork, ethics, dependability, and customer service skills
