Job Description

As a physical therapist, you will be responsible for assessing patient needs and developing comprehensive treatment plans that promote optimal health and well-being in a home care setting.

• You will conduct thorough initial and ongoing assessments of patient condition to determine appropriate treatment plans.
• Administer physical therapy treatments as prescribed by physicians, ensuring the highest level of care and comfort for patients.
• Evaluate treatment outcomes and make recommendations for adjustments or modifications as needed.
• Collaborate with multidisciplinary teams to ensure seamless transitions and continued care for patients after discharge.
