Licensed Practical Nurse (LPN) - Primary Care

Position Summary :
The Licensed Practical Nurse (LPN) provides direct patient care and supports clinical workflows under the supervision of a Registered Nurse and / or licensed provider. This role is essential in supporting CAMcare's value-based care model through team-based delivery, ensuring care quality, population health metrics, and regulatory compliance including HRSA and NCQA standards.
Essential Duties and Responsibilities :
- Provide direct nursing care including :
Vital signs, medication administration, immunizations, blood draws, and point-of-care testing
Assist in patient triage and intake documentation in the Electronic Health Record (eClinicalWorks)with RN / provider oversightAssist providers during exams, minor procedures, and screeningsSupport care coordination by :Facilitating referrals, follow-up appointments, and specialist communication
Monitoring gaps in care reports and proactively reaching out to patientsReview health education to patients regarding chronic conditions, medication adherence, and lifestyle modificationEnsure accurate documentation and compliance with HIPAA, OSHA, and infection control guidelines when assigned, accompanied by a provider health summary.,Maintain inventory of medical supplies and vaccines per policy and cold chain protocolsParticipate in care team huddles, performance improvement activities, and training sessionsKey Performance Indicators (KPIs) :
Clinical Quality Support :Completion of patient intake within 10 minutes of appointment time
Accurate and complete documentation of vitals and health history in EHR ≥ 95%Medication administration error rateOperational Efficiency :Assist in oversight of the medical assistant staff to ensure a consistent workflow in the department.
Daily room turnover timeCollaborate with the vaccine coordinator to ensure the maintenance of vaccine stock logs with 100% monthly audit accuracyPatient Engagement :90% patient satisfaction rating for LPN-related interactions
Documentation of patient interactions in ≥ 90% of eligible encountersCompliance & Participation :Attendance in 100% of required huddles and quality review meetings
Completion of mandatory trainings and licensure renewals on timeHEDIS and Clinical Quality Measures Supported :
LPNs directly support the documentation, coordination, and completion of the following HEDIS (Healthcare Effectiveness Data and Information Set) measures :
Controlling High Blood Pressure (CBP) : Ensure accurate BP readings and re-checks are documentedDiabetes Care - HbA1c Control (CDC) : Assist with lab coordination and patient reminders for HbA1c testingBreast Cancer Screening : Schedule and document completed mammogramsColorectal Cancer Screening : Educate and assist patients with FIT kits or colonoscopy referralsChildhood Immunization Status (CIS) : Administer vaccines per protocol and maintain accurate documentationCervical Cancer Screening : Prepare patients and update records following Pap smear collectionWell-Child Visits (WCV) and Adolescent Well-Care Visits (AWC) : Ensure documentation of developmental screenings and physical examsDepression Screening and Follow-up Plan (DSF) : Administer PHQ-2 / PHQ-9 and document follow-up actionsMinimum Qualifications :
Current and unrestricted LPN license in the State of New JerseyCurrent BLS / CPR certificationMinimum 1 year of outpatient or community health experience preferredFQHC or primary care experience strongly preferredFamiliarity with population health, quality reporting, and value-based care initiatives is a plusProficiency in EHR systems; eClinicalWorks preferredBilingual (English / Spanish) highly desirableCore Competencies :
