Location(s) : Conroe, TX

Specialty : NP treating patients with CKD and polychronic conditions

Support : Will be supported by team of RNs and SWs

Schedule : M-F 8a 5p; 1-2 days working remote; 3-4 days treating patients in-home

Full-time, permanent position

EMR : Athena

Requirements : FNP, AGNP, or ACAGNP

2 years of direct patient care NP experience

Clean and active TX state license

DEA license

BLS
