We are seeking an 0.9 FTE (36 hours per week) Field Nurse Case Manager to cover the Hartford, CT and surrounding area . This position offers full-time benefits.

A Field Case Manager role is a home-based position with travel, up to 2 hours one-way, to doctors’ offices, hospitals, and various other locations. This individual is responsible for the medical case management of work-related injuries, which includes assessment, planning, coordination, implementation, and evaluation of injured / disabled individuals. You must have the ability to operate autonomously with minimal oversight, and the desire to build and navigate relationships with injured workers, providers, insurance companies, employers, and attorneys .

Qualifications :

  • Current, unencumbered Registered Nurse (RN) license in CT
  • Recommended certifications (CCM, CRRN, CDMS, CRC) preferred. REQUIRED within two years of hire date.
  • Valid Driver’s License, and good driving history.
