Duties :
- Performs Patient assessments : Accurately assesses patient's appropriateness for hospice care, learning needs and progress toward goals.
- Develops the plan of care for the patient and family
- Ensures appropriate utilization of services : Maintains visit productivity standards.
- Demonstrates knowledge of insurance reimbursement Completes discharge assessments : Performs laptop transfer of data twice daily.
- Completes admission assessment and transfers within 24 hours.
- Completes re-visit documentation and transfers
- Uses age appropriate teaching methods for patients and families : Evaluates teaching plan and revises as necessary.
- Documents patient education tools used Assumes responsibility for Case Management : Develops / implements / evaluates plan of care based on evidence-based knowledge.
- Revises care plan as indicated.
- Prioritizes care needs.
- Demonstrates knowledge of hospice regulations and insurance reimbursement guidelines.
- Shows evidence of interdisciplinary communication.
- Performs HHA supervisions per standards.
- Ensures appropriate utilization of services.
- Follows scheduling process.
- Communicates progress toward goals with IDT.
- Attends and participates in IDT meetings every 2 weeks.
- Evaluates the plan of care and revises as needed
- Maintains safe environment for patients and co-workers.
- Follows infection control measures and reports infections.
- Demonstrates knowledge of pharmacology and safe medication management.
- Routinely reconciles medications.
- Maintains current medication orders Attends monthly staff meeting : Participates in PI meetings and activities / committees as requested.
- Accepts other assignments as requested.
- Assumes responsibility for case management : Prioritizes schedule to accommodate patient's needs.
- Documents case management activities.
- Demonstrates knowledge of pharmacology and safe medication management.
- Routinely reconciles medications.
- Maintain current medication orders.
- Participates in Performance Improvement Program : Reviews health records and gathers data to support PI activities.
- Attends UR meetings as requested.
- Provides orientation / mentorship to new employees.
- Cross trains to learn new skills and demonstrates a willingness to teach other Assists with growing agency business : Provides orientation to new employees.

This role would be for the evening hours 3p -11p and would require every other weekend commitment.Comp days would be provided during the week when working the weekend.
