State Licensed Patient Care Director

A company is looking for a Director, Patient Care to lead and evolve their Care Navigation and Clinical teams.

Key Responsibilities

Lead and develop a high-performing Patient Care organization

Establish and execute a strategic vision for patient care delivery

Oversee daily operations of Care Navigation and Clinical teams

Required Qualifications

10+ years of experience in patient operations or healthcare service delivery, with 5-8 years in leadership

Active RN license or other clinical licensure preferred

Proven success leading clinical and non-clinical teams

Data-driven with skills in interpreting metrics to improve outcomes

Experience with self-funded employer benefit plans or bundled payment models is a plus
