Job Description :

Sunbelt Staffing is hiring a Home Health RN in Scottdale, PA. The nurse will provide skilled field-based care, manage OASIS documentation, and support patients in their homes. This role is ideal for someone who values independence and patient-focused care.

Key Responsibilities :

Conduct start-of-care assessments and follow-up visits

Document all care in OASIS via Homecare Homebase (HCHB)

Deliver skilled services such as wound care, injections, and chronic condition management

Educate patients and caregivers on care plans and safety

Coordinate with the interdisciplinary team for continuity of care

Qualifications :

Active RN license in Pennsylvania

Minimum of 2 years RN experience, including 1 year in home health

Experience with OASIS and Homecare Homebase (HCHB) is required

Current BLS certification

Valid drivers license, auto insurance, and reliable transportation

Able to pass background check, drug screen, and meet Joint Commission standards
